We are aware of the upcoming pressure you and your teams are going to find yourselves under over winter. 

We will be running 2 online workshops discussing the pressures you experience within your discharge to assess processes and how these are affected by winter pressures. This will be a chance to share your experiences with other local authorities and hopefully learn how other areas are currently working through this process.

As part of this discussion we will look to do some high level mapping of how the process is currently undertaken within your organisations and highlight the pain points you are experiencing. You will also benefit from hearing directly from someone at North Lincs Council who have adopted Hospital to Home. Hospital to Home is a digital solution for your discharge to assess process. This will give you the opportunity to see how their processes have changed through the implementation of this solution. 

The workshop will run for 1.5 hours and once it is finished, we will share all of the outputs of the process mapping, so you have something to take away and share internally with your teams.
