Removing Discharge to Assess (D2A) Complexity

  • Discharge to Assess is a complicated process involving multiple health and care organisations working together to ensure a patient’s safe and timely transfer from hospital to their home or a community setting

  • A single discharge to assess (D2A) process can take anywhere between 3-18 person hours to manage and involve anywhere between 3-100+ people across the local health system

  • Challenges of different systems, multiple hand-offs, reliance on email, telephone and spreadsheets means there is no single source of D2A truth for a local authority or their health and care D2A partners

  • Hospital To Home (H2H) allows D2A professionals to capture, update, track and report on a person’s journey through the D2A process – see the system features here
