We at Tech in Care understand the importance to our customers of the potential for interoperability with existing systems. This is very much on the roadmap for Hospital to Home (H2H) and we are now in the process of finalising the work plan for the first interoperability development with an Acute Trusts Electronic Patient Record (EPR). We will be posting more about this as soon as we have fixed timescales, however throughout the summer we will be working on some developments to prepare H2H for this interoperability work.
The following images show how H2H will evolve the Discharge to Assess process from how things are now, to how it will look with the first phase implementation of H2H, to the next phase of interoperability:
There are already a number of benefits that H2H will deliver for both organisations and patients and we know that interoperability will take these to the next level. It is vital that a process is put into place which will ensure patients do not stay in hospital longer than they need to, and that they get the right support at the right time through their assessments. H2H will continue to drive forward to improve this process for our customers, and interoperability is just one example of how we are looking to do this.
