Co-designed with local authorities and their health and care partners as part of the Local Government Association’s Social Care Digital Innovation Accelerator 2020/2021

Hospital to Home

Removing Discharge to Assess (D2A) Complexity

  • Discharge to Assess is a complicated process involving multiple health and care organisations working together to ensure a patient’s safe and timely transfer from hospital to their home or a community setting

  • A single discharge to assess (D2A) process can take anywhere between 3-18 person hours to manage and involve anywhere between 3-100+ people across the local health system

  • Challenges of different systems, multiple hand-offs, reliance on email, telephone and spreadsheets means there is no single source of D2A truth for a local authority or their health and care D2A partners

Ensuring Patient Safety & Improved D2A Management

  • Hospital To Home (H2H) allows D2A professionals to capture, update, track and report on a person’s journey through the D2A process – see the system features here

  • H2H supports the D2A process from the point at which a patient is identified on the ward as requiring support on discharge, through to the point that the patient receives an assessment of their longer term needs within the community setting

  • Hospital To Home offers the ‘single version of the truth’ crucial not only to offer patient safety, but also bring efficiencies, transparency and an effective digital approach to D2A
